So, I do anterior, bikini,

hip replacements, minimally

invasive for the hip

replacements and as for

the knee replacements,

I usually do a patient

specific instrumented

3D printed, minimally

invasive, knee replacement

with enhanced recovery.

So the whole idea is to enable

them to mobilize early and

discharge home, hopefully

the next day after surgery.

Um, actual journey started

off,  in Sydney in the

early two thousands.

I worked with, my

mentors, Laurie Khan,

professor Khan and, Dr.

Dennis Kerr.

They started the, local

{{00:01:10}}
infiltration analgesia

{{00:01:12}}
technique, which pretty much

{{00:01:14}}
enabled patients to mobilize

{{00:01:16}}
a few hours after surgery

{{00:01:18}}
and go home the next day.

{{00:01:21}}
And this was done almost 20

{{00:01:22}}
years ago and I saw a great

{{00:01:24}}
benefit in this because with

{{00:01:26}}
the patients mobilizing very

{{00:01:28}}
early this actually reduced

{{00:01:31}}
risk of deep vein thrombosis

{{00:01:34}}
and as a result, pulmonary

{{00:01:35}}
embolism and patients felt

{{00:01:37}}
more confident and comfortable

{{00:01:39}}
to get back to normality

{{00:01:40}}
as soon as possible, and

{{00:01:42}}
then I realized that this

{{00:01:43}}
is actually the way to go.

So I started implementing

this, over the last 10 to 12

years since I started, private

practice and I believe this

is the way to go in the years

to come, this will be refined

more and more where patients

are mobilized at day zero,

few hours after surgery

{{00:02:05}}
and more patients

{{00:02:06}}
are, be beginning to

go home the same day.

So the traditional joint

{{00:02:11}}
replacements, per se, that

{{00:02:13}}
have been done for many years.

The idea behind that is

{{00:02:17}}
where patients have surgery,

{{00:02:20}}
the surgery can last for

{{00:02:22}}
one to three hours with

{{00:02:24}}
all the swelling, and pain

{{00:02:27}}
patients usually,  stay

{{00:02:29}}
in bed for overnight,

{{00:02:32}}
from the time of surgery.

{{00:02:33}}
And then they are mobilized

{{00:02:35}}
the next day with that comes

{{00:02:39}}
all the other issues of

{{00:02:41}}
immobility, risk of DVTPE,

{{00:02:45}}
breathing issues and the

{{00:02:47}}
risk of pneumonia because the

{{00:02:49}}
patients are lying in bed.

So the move from

{{00:02:53}}
traditional, to the enhanced

{{00:02:56}}
recovery program or the

{{00:02:57}}
Rapid Recovery Program.

So the whole idea is to

mobilize the patients,

{{00:03:03}}
early, which is two to

{{00:03:04}}
three hours after surgery.

And the whole idea is to send

{{00:03:08}}
them home the next day as

{{00:03:09}}
opposed to going home, after

{{00:03:13}}
a, length of stay in rehab.

So, Three nights, four nights

{{00:03:16}}
in hospital, and then another

{{00:03:18}}
five nights or six nights

{{00:03:19}}
in rehab in the traditional,

{{00:03:22}}
joint replacement program to

{{00:03:25}}
more, rapid recovery program

where, patients are only

{{00:03:29}}
in hospital one night and

{{00:03:30}}
they usually go home with,

{{00:03:32}}
outpatient, physiotherapy,

to enable them to get back

{{00:03:36}}
to normality as soon as

{{00:03:37}}
possible, including, early

{{00:03:39}}
return to driving as well.

I think the rapid recovery

program is actually here

{{00:03:44}}
to stay and it's gonna be

{{00:03:46}}
refined more and more to

{{00:03:47}}
such an extent that I believe

total joint replacement

day case arthroplasty will

{{00:03:53}}
be, a mainstay, for some,

{{00:03:57}}
patients or small component

{{00:03:59}}
of patients but overnight

{{00:04:01}}
stay and discharge home the

{{00:04:02}}
next day will mainly be for

{{00:04:04}}
the majority of the patients.

Very few patients will

{{00:04:07}}
stay in hospital more

{{00:04:09}}
than that, period of time.

With regards to,

technologies, I think

with the technologies and

{{00:04:16}}
techniques, there'll be more

improvements, new techniques,

and new technologies already.

The robotic, technology and

the 3D printed technology.

And a combination of two

{{00:04:27}}
technologies, and I think it's

the third, fourth, and fifth

generation of robotic surgery

that will drive this, rapid

recovery process, looking

into the future and there are

{{00:04:40}}
other technologies including

wearable technologies,

which give us a lot of

data, and I think there'll

{{00:04:46}}
be a lot of investment in

{{00:04:48}}
the wearable technologies

{{00:04:49}}
that give us an idea in the

data that we get from that.

surgery, which will enable
