Utilising digital solutions to enhance transitions of care

In 2017, the third World Health Organization (WHO) Global Patient Safety Challenge – Medication without harm (the Challenge) was launched. The Commission was engaged to develop Australia’s response (the Response) to the Global Patient Safety Challenge. The WHO goal for the Challenge is to reduce severe, avoidable medication-related harm resulting from errors or unsafe practices due to weaknesses in health systems. One of the three flagship areas for the challenge are transitions of care, both within and between the acute sector, primary care and aged care. Digital solutions have the potential to facilitate transitions of care, however unique safety and quality risks are introduced by digital solutions that require consideration. This session looks at how digital solutions can facilitate transitions of care as well as the potential safety and quality risks.
