• Most hospital pharmacy departments use their own local solutions to log discharge prescriptions that need to be dispensed such as paper slips, SharePoint sites and Excel spreadsheets. • These solutions are often designed for internal tracking purposes and not as communication tools between pharmacy and nursing staff. • A novel prescription tracking solution was developed to provide nursing staff with visibility of the dispensing process and hence facilitate discharge planning and patient flow. • The solution utilises existing functionalities within the Cerner Millennium platform and the Ministry of Health's Patient Journey Board to provide real-time updates. • A successful pilot was conducted at the Prince of Wales Hospital and the Wollongong Hospital after which the solution was rolled out more broadly across South Eastern Sydney and Illawarra Shoalhaven Local Health Districts.
