Psychiatry short stay

units have probably been

{{00:00:38}}
around, for two decades.

{{00:00:42}}
Principally they're located in

{{00:00:43}}
Australia, United States, and

{{00:00:45}}
they're designed for, people

{{00:00:48}}
and patients who present

{{00:00:49}}
in crisis with suicidal

{{00:00:50}}
thinking, with depression

{{00:00:52}}
and anxiety disorders.

{{00:00:54}}
And they're traditionally

{{00:00:55}}
co-located next to emergency

{{00:00:57}}
departments, in, mainly

{{00:00:59}}
in the public sector.

And, it's increasingly

{{00:01:02}}
becoming recognized as,

{{00:01:04}}
a proposition that adds

{{00:01:05}}
value to the whole, mental

{{00:01:07}}
health integrated pathway.

Shocks, they pay units

{{00:01:11}}
mainly cater for people

{{00:01:13}}
who present in crisis.

{{00:01:14}}
So you need a crisis

{{00:01:15}}
model of care.

{{00:01:17}}
And often they come in the

{{00:01:19}}
middle of the night, and

{{00:01:20}}
often they, they might need

{{00:01:22}}
a short-term care model in

{{00:01:24}}
a structured setting, like

{{00:01:25}}
a hospital setting, but

{{00:01:26}}
they don't often need to be

{{00:01:27}}
very much longer than that.

So one to two days.

{{00:01:30}}
But the key is to have an

{{00:01:31}}
integrated pathway between

{{00:01:33}}
the ED assessment, the one

{{00:01:35}}
to two days in, in a short

{{00:01:37}}
stay unit, and the post ED

{{00:01:39}}
and short stay unit pathway.

The skill, I guess, is to

{{00:01:42}}
provide that integration and

{{00:01:43}}
that commonality of purpose.

To guide the patient in the

{{00:01:47}}
middle of their crisis to,

{{00:01:49}}
to reduce the intensity of

{{00:01:51}}
their symptoms and offer a

{{00:01:53}}
rehabilitation and a connected

{{00:01:55}}
pathway to community care.

It is correct that there's

{{00:01:59}}
increasing numbers of

{{00:02:01}}
people coming to emergency

{{00:02:03}}
departments, which is

{{00:02:05}}
contributing to issues of

{{00:02:07}}
pressures and demands within

{{00:02:08}}
our healthcare system.

{{00:02:09}}
What we need to firstly

{{00:02:10}}
understand is why people are

{{00:02:11}}
coming to ED and what kind

{{00:02:13}}
of pre-hospital initiatives

{{00:02:15}}
we can set up in primary

{{00:02:16}}
care, secondary care.

{{00:02:18}}
But indeed, once they do

{{00:02:19}}
come to ED, we need to make

{{00:02:21}}
sure that the care pathway is

{{00:02:22}}
streamlined and efficient and

{{00:02:24}}
that they get good quality

{{00:02:25}}
of care that's structured

{{00:02:27}}
and we need to then ensure

{{00:02:29}}
that our connections with

{{00:02:30}}
the GPs, psychologists and

{{00:02:33}}
our community mental health

{{00:02:34}}
themes are integrated and that

{{00:02:36}}
there's good communication

{{00:02:38}}
and that the consumer is also

{{00:02:40}}
involved in the joint decision

{{00:02:42}}
care planning that best meets

{{00:02:44}}
their needs, post-crisis.

{{00:02:47}}
It's very important the design

{{00:02:49}}
of the short stay in it; you

{{00:02:50}}
need to create an environment

{{00:02:51}}
that is, specialized.

{{00:02:54}}
That's, that has a right

{{00:02:56}}
environment and location.

{{00:02:58}}
It's critical that the short

{{00:02:59}}
stay in its are located

{{00:03:01}}
somewhere near the emergency

{{00:03:02}}
department and that there's

{{00:03:03}}
good connections with the ed.

{{00:03:06}}
But clearly there are

challenges for recruiting.

Multidisciplinary team members

allied health nursing within

{{00:03:13}}
our mental health system.

But the key is that we create

a unit that,  people like

to work in and therefore the

retention and recruitment

of new staff is enabled.

And, and I think having

a consistent model of

care will help with that.

In terms of readmission

{{00:03:27}}
rates generally speaking,

{{00:03:29}}
the readmission rates from

{{00:03:30}}
such units tend to be in the

{{00:03:32}}
order of 15% within 28 days

{{00:03:35}}
the importance of reducing

{{00:03:36}}
that is, based on having a

{{00:03:39}}
good pathway in the community

{{00:03:40}}
that connects with the

{{00:03:42}}
short stay psychiatry team.

It's fundamental that the

{{00:03:45}}
patient and the consumer is

{{00:03:46}}
involved in their care plan.

{{00:03:48}}
And we need to set up,

{{00:03:50}}
systems within the short stay.

{{00:03:52}}
That enables shared

{{00:03:53}}
care planning.

{{00:03:54}}
That's critical to the

{{00:03:55}}
conversations that occur

{{00:03:56}}
around the reasons behind the

{{00:03:57}}
crisis, the symptoms, that

{{00:03:59}}
are related to the crisis

{{00:04:02}}
and how we manage them.

{{00:04:03}}
Inevitably, the

{{00:04:04}}
interventions that I involve,

{{00:04:06}}
biopsychosocial frame, may

{{00:04:09}}
involve medication, but more

{{00:04:11}}
importantly, psychosocial

{{00:04:12}}
integration, which includes

{{00:04:15}}
access to social resources.

{{00:04:17}}
Familial resources.

{{00:04:18}}
'cause when people

{{00:04:19}}
are presenting crisis,

{{00:04:20}}
often they feel lonely.

{{00:04:21}}
So the key is to garner the

{{00:04:23}}
support within the existing

{{00:04:24}}
network and if needed,

{{00:04:26}}
organize a therapeutic

{{00:04:27}}
structure on top of that.

{{00:04:28}}
But fundamentally, the patient

{{00:04:29}}
also needs to engage with the

{{00:04:31}}
care plan and understand how

{{00:04:33}}
to manage this symptoms in

{{00:04:36}}
a community setting with the

{{00:04:38}}
guidance of their social and

{{00:04:39}}
or clinical support system.
